To get signed out of ZAP, students will need to complete the assignment with full effort, fill out a green get out of ZAP form located at the office window, have the teacher sign the form, and then return it to the office.

PARENTAL CONSENT FOR STUDENT SURVEY & REQUEST TO COMPLETE FAMILY SURVEY

Thank you to those who have completed the parental consent form to allow their students to the choice to take this important survey. Please make sure after completing the consent form that you take the quick opportunity to complete part 2 and fill the family survey out as well. Thank you!

PART 1: CONSENT FOR STUDENT SURVEY - Parent/Guardian consent for student to complete MRA - Measurable Results Survey at school. If consent to take the survey is given, a student survey will be administered to your student at school during the month of November 2022. PART 2: FAMILY SURVEY - The parent/guardian survey takes about 12 minutes to complete and will ask questions about your child and about his or her experience in our school. If you have more than one child attending the school, you will be asked to think about the oldest child's experiences as you answer questions related to his or her growth. This survey is completely voluntary but we do hope to get feedback from one member of every household. Section 2 of the information below will include the Link for Measurable Results Survey for Parents & Community Patrons to take. Our school is implementing a whole-school transformational process called the Leader in Me, which teaches 21st-century leadership and life skills to students and creates a culture of student empowerment based on the idea that every child can be a leader. Our school will be conducting surveys to learn more about key areas of strength and areas of needed growth in our school. In order for us to have a holistic picture of what is going on in our school, we are asking all full-time staff members, parents/guardians, and students in Grade 7-12 to participate.

We welcome your feedback as we work to continuously improve our schools. This survey is completely anonymous and seeks honest input. Students and families will not be asked for their names on the questionnaire, nor will anyone be able to connect any individual student with his/her responses. School staff will not see any one student's responses but only summaries of results. Participation is completely voluntary. Your child may decline to participate in the survey or may simply skip any questions they do not wish to answer. If there are specific issues we can for you or your family personally, please contact us directly.

INFORMATION ABOUT STUDENT LIGHTHOUSE TEAM & STUDENT LIGHTHOUSE COORDINATORS

We are excited to being the Lighthouse process at Central Jr/Sr High School. Students learned more about this important opportunity in an assembly that presented the information below on Thursday, November 10th. Rise Time Family groups then nominated and voted on their Student Lighthouse representative. Student Lighthouse Coordinator applications are open to all 7th-12th grade students and are due on Tuesday, November 15th.

Student Lighthouse Team Responsibilities:

The goal of the Student Lighthouse Team is to make student’s voices an integral part of all school decisions. Students will collaborate with the Staff Lighthouse Team to accomplish mutually agreed upon goals. The Student Lighthouse team will be an equal partner to the Staff Lighthouse Team and will work together in all initiatives or issues to improve the school community. Some examples of projects or activities the Student Lighthouse team might work on are community service projects, school beautification efforts, expand student leadership roles with new clubs/programs, create student input systems and plan for schoolwide or community events. Ultimately, the goal of the Lighthouse team is to have a positive impact on Students, staff, families and the community.

Student Lighthouse Team members will be made up of one student from each Rise Group. Students must nominate themselves if they are interested in being a Student Lighthouse Member- if more than one student in a group is interested in being a member, then the Rise group will vote on the student they feel would represent their group best. In the event of a tie- the Rise group leaders may choose who they feel is the most appropriate SLT member for their group or notify Ms. Smith/ Mrs. Liebau to determine how the decision will be made.

SLT Members should have the following qualities or traits:

  1. Dependable, Reliable and Respectful

  2. Demonstrate good judgment and decision making skills.

  3. Complete classwork and homework consistently on time.

  4. Possess a strong work ethic and willing to promote leadership among all students in school.

  5. Demonstrate exemplary conduct and citizenship in and out of school and have less than three discipline referrals.

  6. Have good attendance and be willing to participate in meetings or trainings for the Student Lighthouse Team throughout the school year.

  7. Be willing to encourage the learning and living of the 7 Habits through their behavior, student-led promotions, lessons, assemblies and videos.

There will also be an opportunity for two students to apply to be a Student Lighthouse Coordinator. Student Lighthouse Coordinators will serve as the leaders for the Student Lighthouse Team. Any student that has the qualities of an SLT member can apply for the position of Coordinator. Students who are nominated by their Rise group to be a SLT member can apply OR if you were not chosen as the SLT member for your group- you can still apply. Applications will be located in the office. Applications for Student Lighthouse Coordinator will be due on Tuesday November 15th.

The first meeting for Student Lighthouse Team members will be on Tuesday, November 29th, 3rd-7th hour. The Students on the Lighthouse Team will meet with a national Leader in Me Trainer to learn more about what serving on the team means and how they can make an impact at Central. After meeting with the trainer as a student group, both the student and staff lighthouse teams will meet with the trainer to further set goals and create an action plan for moving forward.

This is your chance to be involved and have a voice in the school. Many students have great ideas and want to create change, if you are one of those students please take this opportunity to become a leader!
