Parents please be sure to fill out the Household Income Survey Data Collection Form. Data collected from this survey provides funding to our schools and allows us to benefit from the Community Eligibility Provision (CEP) . This program is the reason we are able to provide free meals for ALL USD 462 students. We receive funding based on a formula that relies on this data.
